Catastrophic Referral - Multiple Traumatic Injuries
- 50 year old male
- Date Referred to Case Management: March 3, 2009
- Date of Injury: March 3, 2009
- Diagnosis: Fracture left wrist, fracture left pubic ramus,
- fracture pelvis, compression fractures of T11, T12, and L1
- Obstacles: Status post IV drug abuser with chronic liver disease, multiple treating physicians, psycho social factors involving non-compliant verbally abusive spouse including threats of litigation
- Treatments: Surgery to left wrist, external fixation device, back brace, PT, DME including walker and crutches, x-rays, CT scans, Percocet, Norco, Ultram, Lyrica, and Tramadol
- Interventions:
- Identification of applicable physicians early in the case
- Trauma physician
- Pain management physician
- Neurosurgeon
- Hand surgeon
- Coordination and monitoring of all diagnostics, DME, and aggressive treatment plans
- Council and education of the injured worker on importance of compliance
- Reduction of pain medication, Percocet and Norco, after 60 days then weaned to Ultram, Lyrica, and Tramadol to reduce narcotic and opioid use or abuse
- Avoidance of cervical surgery through conservative treatment, coordination, and monitoring of functional restoration
- Frequent communication with employer, physicians, injured worker, and claims examiner including avoidance of litigation
- Outcomes: Returned to modify duty on July 1, 2009; Full duty – November 9, 2009
- Declared P&S on December 21, 2009
- Estimated Hard Savings of $71,250
- Estimated Soft Savings of $19,000
