Chronic Pain and UR - Why Would a Reviewing Physician "Approve" Massive Doses of Medications?

UR doctors are repeatedly asked to determine medical necessity about treatment requests for combinations of opioids, benzodiazepines, sleepers, etc., many years after an injury. The worse combinations, and highly suspect for unrestrained drug abuse and addiction, is a combination of 2 different opioids (usually brand names), an anti-anxiety drug, a muscle relaxant, and sleepers to initiate sleep and sleepers to maintain sleep. So when you see a request for Opana® ER, Oxycontin®, Xanax®, Soma®, Ambien® and Lunesta® – look out! We see it all the time, and it always presents a challenge.

For several years, these three cases have dominated the workers' compensation water cooler talk about the 2005 permanent disability schedule. They may even have a Facebook page. (If you’re still at work, wait till you get home to friend it.)
These three cases have centered on the statutory language of Labor Code §4660 that the “schedule … shall be prima facie evidence of the percentage of permanent disability to be attributed to each injury covered by the schedule.” It has been a long-standing principle that when a statute provides a certain fact, group or document is “prima facie evidence”, then it is rebuttable and can be overcome by some other evidence.
